LIABILITY WAIVER & RELEASE FORM

Acknowledgment of Risk

I understand that participation in classes, including high heels dance, involves physical activity and carries inherent risks, including but not limited to slips, falls, muscle strain, injury, or other physical harm. I voluntarily choose to participate and accept all risks associated with these activities.

Health Confirmation

I confirm that I am physically able to participate in this class. I do not have any medical conditions that would prevent my safe participation, or I have consulted a medical professional and received clearance.

Assumption of Responsibility

I agree to listen to the instructor, follow all safety instructions, and modify movements as needed based on my own physical limits. I understand that I am responsible for my own well-being during the class.

Release of Liability

I hereby release and hold harmless the instructor, event organizer, venue, and any affiliated staff from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur during or after participation in this class.

Photo & Video Release (Optional)

I grant permission for photos and videos taken during the class to be used for promotional and marketing purposes, including social media.

Personal Property

I understand that I am responsible for my personal belongings and that the organizers are not responsible for any lost, stolen, or damaged items.
